That fork decal appears on 2005 Six13s. Sometimes the decals are a better indicator than the paint. I would check the catalogs from 2004-2006 and look for matches of components. It's not unusual for Cannondales to turn up with paint that doesn't show in the catalogs.

I am not sure about the range of years of the Siemens bikes, but there is one pictured in the 2006 catalog. It appears that the CAAD designation doesn't appear on the MTBs after 2003 at least in the catalogs.

It's not something I have needed to research. When I looked at my 1987 frame, it had a 2 hole stop, with the holes perpendicular to the center line of the stay. Is there enough material in the stop you removed to allow you to drill another hole to match the existing hole in the frame?
