62cm May 06 1991? Seems to be missing a leading zero on the month.
Had a Schwinn Varsity front wheel.. replaced with a light road wheel. The seat post bolt was buggered. I think the hex socket was stripped out and partially drilled out. Stuck "open" not tight enough to hold the seat post so they added some inch sheet metal screws to hold the post in place. We finished drilling out the damaged bolt and removed the stray screws. All good now. Needs a top tube cable clip. 2 prong plug in style. Small ring was removed. The chain was jammed in the bb when we got it.

Looks like you have it right, 126 spaced 25" T1000 built in June 1991. Too bad it wasn't better taken care of, and it looks like most of the original parts are gone. That said I might be interested in it. All of my Cannondale touring bikes are caliper brake models.
